Effectively responding to the aging of our country's population has a bearing on the overall situation of the country's development, on the well-being of hundreds of millions of people, on social harmony and stability, and is of great significance to the comprehensive construction of a modern socialist country. In order to implement the national strategy to actively respond to the aging of the population, strengthen the work on aging in the new era, and enhance the sense of acquisition, happiness, and security of the elderly, the following opinions are hereby put forward. 有效应对我国人口老龄化,事关国家发展全局,事关亿万百姓福祉,事关社会和谐稳定,对于全面建设社会主义现代化国家具有重要意义。为实施积极应对人口老龄化国家战略,加强新时代老龄工作,提升广大老年人的获得感、幸福感、安全感,现提出如下意见。
1. General requirements 一、总体要求
(1) Guiding ideology. Guided by Xi Jinping's Thought on Socialism with Chinese Characteristics for a New Era, thoroughly implement the spirit of the 19th National Congress of the Communist Party of China and the 2nd, 3rd, 4th, 5th, and 6th Plenary Sessions of the 19th National Congress of the Communist Party of China, strengthen the party's overall leadership of the work on aging, and adhere to Take the people as the center, integrate the development of the cause of aging into the overall planning of the “five in one” and coordinate the promotion of the “four comprehensive” strategic layout, implement the national strategy to actively respond to the aging of the population, and integrate the positive outlook on aging and the concept of healthy aging into the economy and society Develop the whole process, accelerate the establishment and improvement of relevant policy systems and institutional frameworks, vigorously promote the traditional virtues of filial piety and respect for the elderly of the Chinese nation, promote the overall development of elderly care services, health services, social security, social participation, and rights protection, and promote high-quality aging services Develop and embark on a path of actively responding to the aging population with Chinese characteristics.   (一)指导思想。以习近平新时代中国特色社会主义思想为指导,深入贯彻党的十九大和十九届二中、三中、四中、五中、六中全会精神,加强党对老龄工作的全面领导,坚持以人民为中心,将老龄事业发展纳入统筹推进“五位一体”总体布局和协调推进“四个全面”战略布局,实施积极应对人口老龄化国家战略,把积极老龄观、健康老龄化理念融入经济社会发展全过程,加快建立健全相关政策体系和制度框架,大力弘扬中华民族孝亲敬老传统美德,促进老年人养老服务、健康服务、社会保障、社会参与、权益保障等统筹发展,推动老龄事业高质量发展,走出一条中国特色积极应对人口老龄化道路。
(2) Working principles   (二)工作原则

Pay attention to the role of family care and personal self-care, and form a new situation in which multiple subjects share responsibilities, escalated responses to aging risks, and everyone participates in aging businesses. --坚持党委领导、各方参与。在党委领导下,充分发挥政府在推进老龄事业发展中的主导作用,社会参与,全民行动,提供基本公益性产品和服务。充分发挥市场机制作用,提供多元化产品和服务。注重发挥家庭养老、个人自我养老的作用,形成多元主体责任共担、老龄化风险梯次应对、老龄事业人人参与的新局面。
——Insist on systematic planning and comprehensive implementation of policies. Adhere to the combination of responding to the aging of the population with the promotion of economic and social development, adhere to the combination of meeting the needs of the elderly and solving the problem of population aging, ensure that the goals of various policies and systems are consistent, the functions are coordinated, and the cohesion is supported. The doctors, the elderly have something to do, the old have learning, and the old have fun, so that the elderly can share the fruits of reform and development and enjoy a happy old age. --坚持系统谋划、综合施策。坚持应对人口老龄化和促进经济社会发展相结合,坚持满足老年人需求和解决人口老龄化问题相结合,确保各项政策制度目标一致、功能协调、衔接配套,努力实现老有所养、老有所医、老有所为、老有所学、老有所乐,让老年人共享改革发展成果、安享幸福晚年。
——Insist on integrating resources and coordinated development. Build an old-age service system and a health support system that coordinate home and community institutions and combine medical care and health care, vigorously develop inclusive old-age care services, and promote the balanced allocation of resources. Promote the coordinated development of the elderly undertakings and industries, basic public services and diversified services, and coordinate the institutional arrangements for the elderly's economic security, service security, spiritual care, and role play. --坚持整合资源、协调发展。构建居家社区机构相协调、医养康养相结合的养老服务体系和健康支撑体系,大力发展普惠型养老服务,促进资源均衡配置。推动老龄事业与产业、基本公共服务与多样化服务协调发展,统筹好老年人经济保障、服务保障、精神关爱、作用发挥等制度安排。
——Insist on highlighting key points and consolidating the grassroots. Focus on solving the most urgent problems in the elderly's health care for the elderly, insist on ensuring basic, promoting fairness, and improving quality, doing our best and doing what we can to ensure that everyone enjoys basic elderly care and public health services. Promote the lowering of the focus of work for the elderly and the sinking of resources, and promote the gathering of various high-quality service resources to the elderly, their homes, and the surrounding areas, to ensure that the elderly work is handled, the elderly matters are managed, and the elderly are in difficulties. --坚持突出重点、夯实基层。聚焦解决老年人健康养老最紧迫的问题,坚持保基本、促公平、提质量,尽力而为、量力而行,确保人人享有基本养老服务和公共卫生服务。推动老龄工作重心下移、资源下沉,推进各项优质服务资源向老年人的身边、家边和周边聚集,确保老龄工作有人抓、老年人事情有人管、老年人困难有人帮。
2. Improve the elderly care service system 二、健全养老服务体系
(3) Innovate the service model for the elderly in the home community. On the basis of home care, improve the community's elderly care service capabilities through new construction, renovation, lease, etc., focus on the development of the street (township), urban and rural community two-level elderly care service network, and rely on the community to develop diversified home-based elderly care services. The local government is responsible for exploring and promoting the establishment of a model for the extension of professional agency services to communities and families. Sub-district communities are responsible for introducing professional institutions that serve the elderly in terms of meal assistance and cleaning assistance. Community organizations introduce relevant nursing professional institutions to carry out care for the elderly at home; the government strengthens organization and supervision. The government must cultivate professional institutions that serve the elderly, guide their standardized development, and guide them to provide sustained and stable services in accordance with the principle of guaranteed and meager profit. Give full play to the role of community party organizations, explore the “community + property + elderly care service” model, and increase the effective supply of home community elderly care services. Combining with the implementation of the rural revitalization strategy, strengthen the construction of rural elderly care service institutions and facilities, and encourage the development of mutual aid services for the elderly based on village-level neighborhood mutual aid points and rural happiness homes.   (三)创新居家社区养老服务模式。以居家养老为基础,通过新建、改造、租赁等方式,提升社区养老服务能力,着力发展街道(乡镇)、城乡社区两级养老服务网络,依托社区发展以居家为基础的多样化养老服务。地方政府负责探索并推动建立专业机构服务向社区、家庭延伸的模式。街道社区负责引进助餐、助洁等方面为老服务的专业机构,社区组织引进相关护理专业机构开展居家老年人照护工作;政府加强组织和监督工作。政府要培育为老服务的专业机构并指导其规范发展,引导其按照保本微利原则提供持续稳定的服务。充分发挥社区党组织作用,探索“社区+物业+养老服务”模式,增加居家社区养老服务有效供给。结合实施乡村振兴战略,加强农村养老服务机构和设施建设,鼓励以村级邻里互助点、农村幸福院为依托发展互助式养老服务。
(4) Further standardize the development of pension institutions. Various localities should develop institutional pensions through direct construction, commissioned operations, purchase of services, and encouragement of social investment. Strengthen the construction of the Glory Institute. Public elderly care institutions give priority to accepting the disabled (including dementia, the same below), widows, orphans, disabled, elderly people, elderly people in special family planning families, and elderly people who have made important contributions to society, and provide them with quality and safety standards. Of elderly care services. Establish and improve the standards and evaluation systems for elderly care services, and strengthen the supervision of the construction and operation of elderly care institutions. Research and formulate management policies for the advance collection of service fees for elderly care institutions, and strictly prevent acts such as borrowing money from elderly care institutions and fraud.   (四)进一步规范发展机构养老。各地要通过直接建设、委托运营、购买服务、鼓励社会投资等多种方式发展机构养老。加强光荣院建设。公办养老机构优先接收经济困难的失能(含失智,下同)、孤寡、残疾、高龄老年人以及计划生育特殊家庭老年人、为社会作出重要贡献的老年人,并提供符合质量和安全标准的养老服务。建立健全养老服务标准和评价体系,加强对养老机构建设和运营的监管。研究制定养老机构预收服务费用管理政策,严防借养老机构之名圈钱、欺诈等行为。
(5) Establish a list system for basic elderly care services. Localities should formulate a list of basic elderly care services based on financial affordability, and provide appropriate services such as elderly care security, life care, rehabilitation care, and social assistance for different elderly groups such as health, disability, and financial difficulties. The list should clarify the service targets, service content, service standards and expenditure responsibilities, and make dynamic adjustments in accordance with economic and social development and technological progress. Before the end of 2022, a comprehensive assessment system for the abilities of the elderly will be established, and the assessment results will be recognized across departments across the country.   (五)建立基本养老服务清单制度。各地要根据财政承受能力,制定基本养老服务清单,对健康、失能、经济困难等不同老年人群体,分类提供养老保障、生活照料、康复照护、社会救助等适宜服务。清单要明确服务对象、服务内容、服务标准和支出责任,并根据经济社会发展和科技进步进行动态调整。2022年年底前,建立老年人能力综合评估制度,评估结果在全国范围内实现跨部门互认。
(6) Improve the multi-level old-age security system. Expand the coverage of endowment insurance and gradually realize full coverage of statutory personnel in basic endowment insurance. The national overall plan for the basic endowment insurance for enterprise employees will be realized as soon as possible. Improve the basic old-age insurance benefit adjustment mechanism to guarantee the basic life of benefit recipients. Vigorously develop enterprise (occupational) annuities, and promote and standardize the development of the third pillar endowment insurance. Explore ways to increase the income of rural elderly people through asset income support systems.   (六)完善多层次养老保障体系。扩大养老保险覆盖面,逐步实现基本养老保险法定人员全覆盖。尽快实现企业职工基本养老保险全国统筹。健全基本养老保险待遇调整机制,保障领取待遇人员基本生活。大力发展企业(职业)年金,促进和规范发展第三支柱养老保险。探索通过资产收益扶持制度等增加农村老年人收入。
3. Improve the health support system for the elderly 三、完善老年人健康支撑体系
(7) Improve the health service and management level of the elderly. Strengthen the publicity and education of elderly health knowledge in urban and rural communities to improve the health literacy of the elderly. Do a good job in the health management of the elderly in the national basic public health service projects and the health management services of traditional Chinese medicine. Strengthen the early screening, intervention and classification guidance of key chronic diseases of the elderly, and carry out the oral health of the elderly, the nutrition improvement of the elderly, the prevention and treatment of Alzheimer's and psychological care. Increasing the coverage rate of contracted services by family doctors for the elderly who are disabled, seriously ill, the elderly, and low-income, etc., and improve the quality of services. Expand the scope of home medical services provided by the medical consortium, such as family beds and outpatient visits. Reimbursement of related medical expenses can be made according to regulations, and on-site service fees will be charged at cost. Actively play the role of primary medical and health institutions in providing quality Chinese medicine services to the elderly. Strengthen the construction of the National Geriatrics Center and deploy several regional geriatric medical centers. Strengthen the construction of geriatric medicine departments in general hospitals. By 2025, more than 60% of general hospitals will set up geriatric medicine departments in second-level and above general hospitals. Strengthen the construction of geriatric hospitals, rehabilitation hospitals, nursing homes (centers, stations) and special care hospitals through new construction, reconstruction, expansion, and transformation and development, and establish a two-way referral mechanism for medical care, rehabilitation and nursing care. Speed up the construction of friendly medical institutions for the elderly to make it easier for the elderly to see a doctor.   (七)提高老年人健康服务和管理水平。在城乡社区加强老年健康知识宣传和教育,提升老年人健康素养。做好国家基本公共卫生服务项目中的老年人健康管理和中医药健康管理服务。加强老年人群重点慢性病的早期筛查、干预及分类指导,开展老年口腔健康、老年营养改善、老年痴呆防治和心理关爱行动。提高失能、重病、高龄、低收入等老年人家庭医生签约服务覆盖率,提高服务质量。扩大医联体提供家庭病床、上门巡诊等居家医疗服务的范围,可按规定报销相关医疗费用,并按成本收取上门服务费。积极发挥基层医疗卫生机构为老年人提供优质中医药服务的作用。加强国家老年医学中心建设,布局若干区域老年医疗中心。加强综合性医院老年医学科建设,2025年二级及以上综合性医院设立老年医学科的比例达到60%以上。通过新建改扩建、转型发展,加强老年医院、康复医院、护理院(中心、站)以及优抚医院建设,建立医疗、康复、护理双向转诊机制。加快建设老年友善医疗机构,方便老年人看病就医。
